This Parent/Guardian Consent Declaration (“Consent”) applies to the enrollment and participation of me personally or a minor child under my parental or guardian custody in any Academy Program category (athletes, parents resources where the child participates, coaches-in-training where a minor participates, teams/group environments, and individual high-performance athlete support) and services provided under the Get Better brand, operated by Albena Branzova.
Services may include:

Coaching sessions (1:1 or group)

Programs such as The Next Play Confidence

Camps and immersion events
-
Coach development and certification
-
Digital materials
By participating, I confirm that I understand and agree to the terms outlined below.
2. Nature of Services
I understand that the services provided are focused on:

Athletic development

Personal growth
These services are:

Educational and performance-based

Not medical, psychological, or therapeutic treatment
3. Voluntary Participation
I confirm that:

My participation (or my child’s participation) is voluntary

I am choosing to engage in this process willingly
4. Personal Responsibility and Authority
I confirm that I am the child’s parent or legal guardian and that I have the sole legal authority to provide the consents in this document on behalf of the child. I declare that I have secured any additional consents that may be required by the another parent or legal guardian that is required and entitles me to sign this declaration of consent.

6. Media & Promotional Use (Optional Consent)
Separate permission may be requested for:

Photos or videos

Promotional or educational use
No content will be shared publicly without explicit additional approval.
7. Physical Activity Acknowledgment
For camps, training sessions, and physical activities:

8. Medical Responsibility

9. Minors (Under 18)
If the participant is under 18:

10. Conduct & Participation
